**Join Our Team as a Registered Nurse Case Manager!** Job Description:- Start date: ASAP- Shift: Day 5x8-Hour (08:00 - 16:30)- Patient Ratios: 1:20- Floating Expectation: No- Years of Experience Required: 2-year acute care Hospital Case Manager experience in Care Coordination and Discharge planning - First-time travelers accepted: Yes — Clinical will screen first-time travelers- Weekend Requirement: Every other weekend + meet department needs- CA state license: Yes- Pending License accepted: Yes, but must be licensed upon start date- RTO Restrictions: No more than 7 days Key Responsibilities:- Exciting opportunity in an Acute Care Facility where you will: - Engage in dynamic patient care coordination and discharge planning - Work with diverse patient age groups, including adolescents, adults, and geriatrics - Address special patient needs such as drug/alcohol abuse treatment options, mental health crises, and homelessness - Collaborate with an interdisciplinary team including Social Services and Transportation - Facilitate family meetings and participate in the development of treatment plans - Utilize InterQual Criteria and ensure compliance with CMS and DRG regulations - Manage medical equipment arrangements (DME) and transportation independently Required Skills:- 2-year acute care Hospital Case Manager experience - Proficient in computerized documentation (Epic required)- Competent in Microsoft applications- Knowledge of care coordination, discharge planning, and regulatory criteria Why Catalytic Solutions (CatSol): At CatSol, we connect dedicated therapists with rewarding assignments across the country.
