Requirements- Coordinate and manage inpatient care plans to ensure timely, safe discharges
- Perform utilization review and case management to optimize patient placement
- Lead discharge planning and needs assessments, including DME orders
- Document thoroughly in Epic to maintain accurate, compliant charts
- Collaborate with interdisciplinary teams (PT, RT, pharmacy, social work) to streamline care
- Conduct audits and DRG reviews to support regulatory compliance
- Float between units as needed within scope to meet facility staffing demands
- Serve as charge nurse/lead when required and support unit leadership
Qualifications
- Washington RN license or Compact RN license required
- BLS certification required
- Minimum 2 years recent inpatient acute care hospital experience (within last 3 years) — HARD STOP
- Previous charge experience required
- Epic charting experience required
- Demonstrated experience with care coordination, discharge planning and utilization review
- Ability to work EOW (every other weekend) and float between units as needed
- Pending licenses not accepted; local candidates not accepted (50-mile local radius rule applies)
