Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting personalized treatment plans
• Implement physical therapy interventions designed to enhance mobility, strength, balance, and overall function
• Work in tandem with nurses, doctors, case managers, and other rehabilitation professionals to maximize patient outcomes
• Provide education for patients and families regarding therapeutic goals, discharge planning, and essential home exercises
• Meticulously document evaluations, treatments, and progress per facility and regulatory protocols
• Engage in multidisciplinary care rounds and conferences to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized institution

Certifications

• Valid State licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (e.g., neurological or geriatric) are advantageous but not mandatory

Skills

• Preferred prior experience in the field
• Ability to work with patients presenting complex medical and mobility challenges
• Demonstrated strong communication and collaborative skills
• Familiarity with digital charting and clinical documentation systems is essential
