Description- Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting personalized treatment plans
- Implement physical therapy interventions designed to enhance mobility, strength, balance, and overall function
- Work in tandem with nurses, doctors, case managers, and other rehabilitation professionals to maximize patient outcomes
- Provide education for patients and families regarding therapeutic goals, discharge planning, and essential home exercises
- Meticulously document evaluations, treatments, and progress per facility and regulatory protocols
- Engage in multidisciplinary care rounds and conferences to ensure cohesive patient care
- Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized institution
- Valid State licensure as a Physical Therapist
- CPR certification is preferred
- Specialty certifications (e.g., neurological or geriatric) are advantageous but not mandatory
- Preferred prior experience in the field
- Ability to work with patients presenting complex medical and mobility challenges
- Demonstrated strong communication and collaborative skills
- Familiarity with digital charting and clinical documentation systems is essential
