Description- Conduct thorough evaluations of patients across inpatient or outpatient settings and curate personalized treatment plans
- Implement various physical therapy techniques focused on enhancing mobility, strength, balance, and overall functionality
- Work collaboratively with medical teams including nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
- Inform patients and their families on treatment objectives, discharge plans, and home exercise regimens
- Carefully document evaluations, interventions, and patient progress in alignment with facility protocols and standards
- Engage in interdisciplinary rounds and discussions to promote holistic patient care
- Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution
- Current state licensure as a Physical Therapist
- Preferably CPR certified
- Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
- Strongly preferred prior clinical experience
- Ability to competently address patients with complex health conditions and mobility challenges
- Excellent communication skills and a strong sense of teamwork
- Familiarity with electronic medical records and clinical documentation software
