Description- Conduct evaluations for patients in inpatient and outpatient environments and develop tailored treatment plans
- Implement therapeutic interventions focused on mobility, strength, balance, and overall functionality
- Work closely with interdisciplinary teams, including nurses, physicians, case managers, and rehabilitation specialists to ensure optimal results
- Guide patients and their families through therapeutic objectives, discharge planning, and home exercises
- Maintain detailed documentation of evaluations, treatments, and patient progress as per facility and regulatory standards
- Engage actively in care team discussions and interdisciplinary conferences to guarantee synchronized care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program
- Valid state licensure as a Physical Therapist
- CPR certification preferred
- Specialized certifications (e.g., in neurology, geriatrics) are advantageous but not mandatory
- Prior experience is strongly preferred
- Ability to treat patients with intricate medical and mobility challenges
