Requirements
Required Qualifications
- MD or DO degree
- Completion of an ACGME-accredited cardiovascular disease fellowship
- Board Certification
- Active Oklahoma medical license or eligibility to obtain one
- Current DEA registration and OBNDD certificate
- Clean professional record
