Compensation & Benefits:

• Base Salary - $350,000 Annually
• Bonus of 28% of Annual Personal Professional Net Collections over 3.0 X Base Salary

• 5 MD (Ophthalmologists), 12 OD (Optometrists), 8 Locations, + 2 Ambulatory Surgery Centers
• OD First Call, No Hospital Call
• Services Include: Cornea, LASIK, Cataract Surgery, Glaucoma Management and Surgery, Medical Retina, Eye Lid Surgery, Optometry, Dry Eye, and Optical
• Currently Expanding Locations and Physicians Throughout Mid-Florida
• 3 Physician Owners, with Partnership Track for New Ophthalmologist Based on Existing Formula as Well as Personal Production, Time, and Owner’s Discretion
