Tomah Memorial Hospital
EOE
POSITION DETAILS:
 Weekender with benefits; Licensed Practical Nurse (LPN) only position, primarily Urgent Care but may assist in ED as needed; works every other weekend, Saturday and Sunday, 9a-5p, plus 3-4 additional weeknight shifts, 4:30p-10p, for a total of about 36 hours per pay period.  May include holiday rotation.  Every other weekender guidelines apply.
 JOB SUMMARY:
 
Clerical
Under the supervision of the Registered Nurse, performs a variety of clerical duties associated with patient care unit operations. The LPN will comply with objectives, standards and policies of the department and within the parameters of his or her preparation. Cooperates with the members of the nursing and medical staffs and other hospital departments.
Clinical
Under the direct supervision of an RN the LPN performs assigned nursing care functions following hospital and nursing standards under the general supervision of the clinical director.
 
MAJOR JOB FUNCTION:
 
Clerical
1. Maintains an orderly and businesslike environment at the nursing station in order to assist members of the health care team in the performance of their jobs despite high stress factors.
2. Transcribes doctors' orders per hospital policy. Prepares and maintains computer information. Completes daily order entry of the electronic medication administration record. Notifies RN immediately of all STAT orders. Updates patient Kardexes daily and with each new order.
3. Stickers, assembles, completes and processes patient's charts as indicated. Maintains an accurate clinical record, recording information accurately per hospital policy.
4. Audits charts daily for incomplete or missing documentation (i.e. Progress Notes, Care Plans, Education Plans, Safety Assessment, Discharge Summaries, Medication Reconciliation, unapproved abbreviation and critical lab documentation), brings same to the attention of the appropriate member of the health care team for correction. Checks charts and removes non-permanent reports (if no notations on it) when permanent reports are obtained.
5. Initiates discharge instructions in Exit Care system.
6. Communicates with members of the health team, patients and visitors using verbal and written means utilizing equipment such as telephone, paging system and computer. Performs message duties as necessary and will also complete requests for non-medical services for patients.
7. Is responsible for updating patient activity sheet for each shift. Prepares daily patient service requisitions. Sends profile and change-of-bed status to pharmacy on patient admission, discharge and transfer.
8. Initiates daily worksheets, such as Intake & Output, daily flow sheets, charge sheets and is responsible for obtaining lab and x-ray reports and notifying RN of results.
9. Prepares special forms such as body-release permit. Obtains closed records from HIS as needed.
10. Orders unit supplies to maintain inventory of the unit's equipment and stock and supplies. Prepares requisitions and reports for maintenance repairs.
11. Completes patient call backs on a daily basis.
12. Attends staff meetings at least 50% of the time. Reads and initials all meeting minutes of those not in attendance.
13. Maintains a good working relationship among all nursing unit personnel and other hospital employees through appropriate communication. Willing to identify problems and participate in problem solving process. Demonstrates the ability to evaluate suggestions and criticism objectively.
14. Accepts reassignment as needed to provide necessary coverage for all patient care units.
15. Demonstrates proficiency in regards to the computer documentation of patient cares and any other related computerized patient information/documentation.
16. Performs other duties as assigned.
 
Clinical
1. The LPN will always functions under the direct supervision of the Registered Nurse. Direct supervision means immediate availability to continually coordinate, direct and inspect firsthand the practice of another (N6.02 (6) WI Adm. Code). The Registered Nurse may delegate any act within the scope of professional nursing to the LPN, providing the Registered Nurse does the following: a) Delegates tasks commensurate with educational preparation and demonstrated abilities of the Nurse Intern, b) Provides direction and assistance to the Nurse Intern/LPN c) Observes and monitors the activities of the LPN, d) Evaluates the effectiveness of acts performed by the Nurse Intern/LPN e) Assesses and evaluates patients and patient care.
2. The LPN shall not perform the functions of patient assessment, receiving MD orders, ordering prescriptions, signing consents and giving patient's discharge instructions. Those responsibilities MUST be done in conjunction with the Registered Nurse

EDUCATIONAL REQUIREMENTS
 
1. Current and valid LPN license in the State of Wisconsin
2. Current certification or eligibility to become BLS certified.
3. Medical Terminology knowledge required.
 
QUALIFICATIONS/SKILLS:
 
1. Excellent verbal and written communication skills.
2. Excellent computer skills.
3. Excellent human relation skills as demonstrated by the ability to interface positively with all customers and employees.
4. Must complete a minimum of 8 hours of eMAR training with Super User and 2 hours 1:1 with Pharmacist, complete medication safety class, and maintain ongoing competency.
5. Must acquire the knowledge and maintain established competency levels to perform all other essential duties.
6. Must have proficient computer skills with the ability to operate within a Windows environment.
7. Must have the ability to work with frequent interruptions, under stress and with minimal supervision.
8. Needs to possess as high level of intelligence and integrity, sense of responsibility and ability to comprehend and analyze details.
9. Ability to operate and handle many kinds of complex equipment.
10. Ability to demonstrate clinical competence and maintain current knowledge in caring for infant through geriatric patients.
 
JOB REQUIREMENTS
 
1. Regularly required to use hands to finger, handle or feel objects, and talk and hear.
2. The LPN is frequently required to stand, walk and reach with hands and arms.
3. The LPN is occasionally required to sit, climb or balance, stoop, kneel or crouch.
4. The LPN must occasionally lift and/or move up to 50 pounds.
5. Good hearing is necessary to receive detailed information through oral communication and to make fine discriminations in sound.
6. Visual acuity is needed to assess color changes, to verify accuracy of written materials.
