Position Summary:
A Transition Program Coordinator is typically responsible for helping individuals, especially students or young adults, transition from one phase of life to another. This could include transitions from school to work, high school to college, or juvenile facilities to society. Their duties often overlap with case management responsibilities, where the focus is on assessing the needs of individuals, connecting them to necessary services, and monitoring progress.
Duties & Responsibilities:
THIS POSITION IS LOCATED ON ARKANSAS STATE UNIVERSITY - MOUNTAIN HOME CAMPUS
- Assessment and Planning. Needs Assessment: Conduct assessments of clients' needs to understand their unique challenges and strengths.
- Individualized Transition Plans: Develop personalized plans for each client based on assessment results. These plans often include goals related to education, employment, housing, and other essential life areas.
- Develop Support Strategies: Create strategies to address barriers to success, including academic, social, or emotional support. Connect clients with appropriate resources, such as job training, counseling, academic support, housing assistance, or legal services. Work closely with community organizations, employers, university partners, and other service providers to ensure clients receive comprehensive support. Monitoring Progress.
- Track Client Progress: Regularly monitor the progress of clients toward their transition goals.
- Adjust Plans as Needed: Make modifications to transition plans if clients face new challenges or require additional support. Conduct periodic reviews of client cases to ensure services are still effective and clients are moving forward in their transition. Ensure all case files are up-to-date with relevant information on client goals, interventions, and outcomes.

Minimum Qualifications:
Masters in Rehabilitative Services/Counseling, School Psychology, Social work, Psychology
